The central record for everyone you provide care to. Each client has a profile holding their details, care plan, risk assessments, medication, visit history, documents, alerts and family contacts.

Where to find it: Care Management → Clients

Add a new client

  1. Go to Care Management → Clients.
  2. Click 'Add Client' (top right).
  3. Enter their name, date of birth, NHS number, address and contact details.
  4. Save. You can then open the client to build their care plan and risk assessments.

Build or update a care plan

  1. Open a client and select the 'Care Plan' tab.
  2. Each of the 20 person-centred categories (personal care, mobility, nutrition, etc.) can be edited individually.
  3. Click a category, fill in the Need, Goal, Support and Outcome, set a review date, and save.
  4. Every save creates a version snapshot so you keep a full history.

Record a risk assessment

  1. Open a client and select the 'Risks' tab.
  2. Choose a risk category (falls, medication, moving & handling, etc.).
  3. Set the risk level and record the hazard, who is at risk, and control measures.
  4. Save to snapshot the version and set a review date.

Add a client alert

  1. Open a client and use the Alerts panel at the top of the Overview tab.
  2. Add an alert with a severity (red, amber, blue), a title and detail.
  3. Active alerts show on the client overview and on the carer's mobile app at the visit.

Export a CQC care record pack

  1. Open a client.
  2. Click 'Export CQC Pack' in the header.
  3. A printable document opens with the care plan, risks, alerts and staff acknowledgements — use your browser's Print to save as PDF.

Tips & good to know

  • Use the Smart Assessment tab to turn a structured intake into proposed care plans, risks, monitoring and alerts you can review and publish.
  • Add a client photo from the header so staff can recognise who they are visiting.
